Charles University (Medicine - Dentistry)

1. A photocopy of the applicant’s passport or identity card.
2. A photocopy of FCE (Lower Proficiency in English) or PCE (Certificate of Proficiency in English) diploma.
3. A copy of school-leaving certificate for Secondary Education (if the applicant is in the final year of Secondary School, he must present the grades for the last semester or from the previous year and before the entrance exams in July, he files a copy of the school- leaving certificate).
4. A completed (filled-in) application form (filled and submitted by CMS Agency to the relevant University).

In order for the candidates to take part in the entrance exams, all entry requirements should be delivered to our offices in order to be sent to the University together with the application form.
Charles University (Pharmacy)

1. Secondary school certificates (study reports from each study year, list of subjects with number of lessons, leaving certificate, A- Levels).
2. Either Czech Embassy or Ministry of Education of the country of studies can do official validation. (Do not provide originals but copies officially verified by a solicitor, barrister or notary – verification by school principals is not acceptable!)
3. Two photographs (passport size)
4. Certificate of medical fitness
5. Curriculum vitae
6. English certificate TOEFL 50-70 or IELTS 5.5 – 5.0.

All entry requirements should be delivered to our offices in order to be sent to the University together with the application form.
Charles University (Faculty Of Physical Education & Sports)

1. Two passport-sized, coloured, photographs.
2. A verified copy of secondary/high school certificates and leaving certificate and official transcript, including all marks/grades. This copy should be officially translated into English and must be certified with an Apostille.
3. A true copy of FCE (First Certificate in English/ Lower) or PCE (Certificate of Proficiency in English) diploma, certified by a lawyer or a Citizen Service Centre.
4. A Health Certificate from an authorized medical institution, in English or officially translated, verifying that the applicant is healthy and doesn’t suffer from HIV or Hepatitis.
5. A Curriculum Vitae (CV) – in English.
6. Two letters of recommendation from Secondary School teachers (Chemistry & Biology), translated into English.

All entry requirements should be delivered to our offices in order to be sent to the University together with the application form.
Charles University (Physiotherapy)
